Registration & Immunization

We welcome new students and their families to Santa Cruz Elementary School. You will find a supportive community of teachers, parents, and community members ready to help your child succeed.


To register your child for school, please bring the following documents to our office:

  • Original birth certificate
  • Immunization records or exemption
  • Proof of residency in district:
    • *Two utility bills (electric, water, or telephone)
    • *Lease, rental agreement, or proof of home ownership
  • Picture identification that shows a parent or guardian’s legal address
  • Withdrawal forms and report card or transcripts from previous school
  • Proof of custody if parents are separated or divorced


Arizona state law requires that all students be immunized before attending school. Students who have not been immunized will be excluded from school in the event of an epidemic. For students who require an exemption from immunization, contact the school nurse in the health office.
